Looks like I had trouble with this place years ago but it must've been my error. Dr. M is the best. This is who to call for a new card or renewal, period. No need to miss work for a card renewal. Ultra fast too, I had my AZDHS approval just a few days after I initiated the process. Previous doctors have forced me to let them file for me, which means they need a copy of my card details, and that they'll get to it in a week or two. Not this place, I was done in minutes, no sweat. Highly recommended.
